THIS NOTICE DESCRIBES HOW MEDICAL INFORMATION ABOUT YOU MAY BE USED AND DISCLOSED AND HOW YOU CAN GET ACCESS TO THIS INFORMATION. PLEASE REVIEW IT CAREFULLY.

This notice covers the health information European Wellness Institute, LLC (“EWI”, “we”) creates or receives while caring for you, and everyone on our team who handles it.

How we use and share your health information

Treatment. Our clinicians use your information to care for you and may share it with other providers involved in your care, such as a laboratory or your own doctor.

Payment. To bill you and record your payments.

Operations. To run the practice: scheduling, quality review, training and contacting you about your program. Companies that help us (for example, our record, email and billing systems) must sign agreements to protect your information.

Other uses the law allows or requires. When legal conditions are met, we may share information:

for public health and safety (reporting disease, adverse reactions, suspected abuse or neglect) or to prevent a serious threat to health or safety;
for health oversight, research with privacy safeguards, or organ donation requests;
with a coroner, medical examiner or funeral director;
for workers' compensation, law enforcement or special government functions;
in response to a court order, subpoena or other legal process;
when federal or state law requires it, including to the U.S. Department of Health and Human Services.

People involved in your care. Unless you object, we may share relevant information with a family member or friend helping with your care, or for disaster relief. If you cannot tell us your wishes, we will use our professional judgment.

Uses that need your written permission. We will not use or share your information for marketing, sell it, or share psychotherapy notes without your written permission. Any other use not described here also needs it. You may withdraw permission in writing at any time, except for what we have already done in reliance on it.

Where state law gives you more protection than this notice, we follow the stricter rule.

Your rights

Contact our Privacy Officer (below) to use any of these rights.

See and copy your records, on paper or electronically. We respond within 30 days and may charge a reasonable, cost-based fee.
Correct your records if you believe they are wrong or incomplete. If we say no, we explain why in writing within 60 days.
Ask us to limit what we use or share. We do not have to agree, except that if you pay for a service in full yourself and ask us not to share it with your health plan, we will agree unless the law requires otherwise.
Ask for confidential communications, such as a particular way or address to reach you. We agree to reasonable requests.
Get a list of disclosures from the past six years, other than for treatment, payment, operations and a few other cases. One list a year is free.
Get a paper copy of this notice at any time, even if you received it electronically.
Choose someone to act for you. A person with your medical power of attorney or your legal guardian can use these rights for you once we confirm their authority.
Complain (see below).

Our duties

We are required by law to keep your health information private and secure.
We must follow this notice and give you a copy of it.
We will tell you promptly if a breach occurs that may have compromised your information.
We may change this notice; changes apply to all information we hold. The current notice is available at our office, on our website and on request.

Complaints

If you believe your privacy rights have been violated, you can complain to our Privacy Officer, or to the U.S. Department of Health and Human Services, Office for Civil Rights: 200 Independence Avenue SW, Washington, D.C. 20201; 1-877-696-6775; www.hhs.gov/ocr/complaints. We will not retaliate against you for filing a complaint.
